CELIA is not intended to replace diagnoses and/or treatment from a medical care provider (MD, DO, NP, PA) As such, CELIA is not a physician or psychologist and the scope of services should not be deemed as medical advice. 

The focus of CELIA is to provide customized wellness programs, workshops and education on psychedelic services for introspective purposes. Program experiences and benefits may include spiritual, mental and/or physical health improvements. CELIA will aim to achieve optimal health results, dependent on the participants goals and needs but cannot promise or guarantee results. When consulting on the topic of illegal substances, it is at the discretion of the participant to know the laws pertaining to said substance use. Participants knowingly enter into the program at their own risk. 

It is up to the participant to disclose all relevant medical history for appropriate consultation and individualized programming and fully accepts responsibility if past medical history is misrepresented. Disclosures of drug use including but not limited to prescription medications, over the counter supplements and illicit supplements is solely intended to be complete history. Any adjustments to prescriptions/dosages should be made under the care/consultation of the participants' medical provider. 

By signing below, I acknowledge that CELIA is not a medical provider. In the event medical conditions arise, I as a participant will actively notify and seek treatment for said conditions from a licensed medical professional. CELIA will not be held liable for failure to diagnose or treat an illness and will not be liable for failure to prevent future illness.
